Overview about the Patient Chart

Last reviewed August 20, 2026

The Patient Chart is the central hub for all content and activities related to a specific patient.

This page explains the main ways of accessing the patient chart, the different sections, and how to navigate around.

Accessing the Patient Chart

Here are common ways to access a patient’s chart:

  • After creating a new patient
  • Search and select from Patient List application
  • Open patient chart shortcut
  • Click on the patient header in the appointment preview

While users can have multiple patient charts open at any given time, each tab represents a unique patient.

Patient chart layout

The workspace for the patient chart is comprised of three main areas:

  • Patient header - Display key patient information and is always visible at the top
  • Chart workspace - Dynamic view based on the chart navigation or workflow.
  • Chart navigation - left side navigation within the chart workspace that is the primary way to navigate the patient chart. The chart navigation may not be visible in certain workflows.

Patient header

The patient header allows users to always know which patient chart they are viewing. The patient header is always visible at the top of the chart and displays patient information such as patient name, date of birth, age, gender, and MRN.

An indication will also display if the patient is deceased or if the record is considered inactive.

Chart workspace

The Chart workspace is the large area below the patient header. It is a dynamic view based on what the user is viewing or doing based on the chart navigation, e.g. viewing or adding new clinical data, documenting an encounter, messaging a patient, or sending a consult note.

Chart navigation

The chart navigation is the left side navigation within the chart workspace which is the primary way to navigate the patient chart.
