Recording and Managing Vitals

Record, edit, and review patient vitals in the Clinical section of the Patient Chart.

This guide walks you through recording, editing, and reviewing a patient’s vitals Clinical section of the Patient Chart.

Before you start

You’ll need to have a patient chart open. The Vitals table lives under the Clinical section of the Chart Navigation.

How to record a new vital

  1. Open the patient’s chart and go to the Vitals table.
  2. Select Add Vital. This opens the vitals modal.
  3. Option to enter vitals in Imperial (°F, in, lb) or Metric (°C, cm, kg).
  4. Record vitals. Date and time should reflect the time when the vitals were conducted.
  5. Select Save.

Vitals flowsheet

The Vitals flowsheet lists the vitals on the left-most column with the recorded dates across the top.

Toggle between Newest first and Oldest first to adjust the date sort order

Supported vitals

  • Temperature
  • Heart rate
  • Blood pressure (systolic / diastolic)
  • Height
  • Weight
  • BMI
  • SpO2
  • Respiratory rate

Age-specific vitals

  • 0-36 months
    • Head circumference
    • Head circumference percentile
    • Weight to length percentil
  • 2-20 years
    • BMI percentile

Physiological ranges by age

To improve data entry quality, below is the table of accepted ranges per vital sign and common data entry mistakes. Values outside of these ranges are considered physiological impossible.
