Create a New Patient
Create a new patient record and enter the required information in OpenCoreEMR.

Create a patient record and continue to the chart once the required information has been entered.
Before you begin
- You must have access to the Patients application.
- You may need permission to create patient records in the system.
- If a matching patient already exists, review the duplicate-check results before creating a new record.
Create the patient
Create new patients from either the Patients application or Patient Search.
- Open Patients and select Create new patient.
- Enter the patient details.
- Required fields include First name, Last name, Date of birth, and Sex.
- Optional fields may include Middle name and address information.
- As you enter the details, a section below displays existing patients with similar information to help prevent duplicate records.
- Select an existing patient if a match is found, or create a new patient if no matching record is found.
- Continue to the patient chart after the record is created.
Confirm the result
The new patient record is saved and the user is directed to the patient’s chart.
If similar records exist, the duplicate-check section helps staff confirm whether the patient already exists before creating a duplicate record.
Troubleshooting
- If the duplicate-check section shows similar patients, review the matching records before creating a new entry.
- If required fields are missing, complete the required information and save again.
